Gifts by Bequest

A gift in your will is a simple way to create a legacy that will support hospice patients and families for years to come.

 

Below is wording that may assist you and your attorney in drawing up a bequest to benefit Trellis Supportive Care. Each situation is unique – these are only suggested examples. We encourage you to notify us of your intent so we can best ensure that your wishes are honored.

Your gift may be a specific dollar amount, a particular asset, a percentage of your estate, or the remainder after all other provisions are specified.

General bequest

The following language will allow Trellis Supportive Care to use funds for general purposes and whatever needs are deemed most pressing.

“I give to Trellis Supportive Care, a charitable organization currently located at 101 Hospice Lane, Winston-Salem, NC, 27103, __________ [outright gift amount/description of asset/percentage of estate] for its unrestricted charitable use and purpose.”

Specific bequest

The following language will designate your provision for a particular purpose.

“I give to Trellis Supportive Care, a charitable organization currently located at 101 Hospice Lane, Winston-Salem, NC, 27103, __________ [outright gift amount/description of asset/percentage of estate]. This gift should be used for ________________ [state purpose], if and so long as the need exists. If circumstances should change at some future time and make it impossible or impractical to administer the gift for the purpose designated, then Trellis Supportive Care may redesignate the use of my bequest, adhering as closely as possible to my original intent.”
