Improving Cardiac Care through Collaboration
We are pleased to announce a collaboration with the American Heart Association (AHA) and the National Partnership for Healthcare and Hospice Innovation (NPHI) to improve the quality of life for patients and families living with heart disease. The collaboration means that cardiac patients can receive advanced cardiac treatments and therapies, in their home. Heart disease is the leading cause for hospitalization: patients with congestive heart failure account for more than one million inpatient admissions annually. Despite the longer survival times for heart failure patients enrolled in hospice than those not enrolled, services remain underutilized for this population.
In-home specialized therapies will include IV diuretics, milrinone and other Inotrope infusions, and LVAD therapy. Patients have 24/7 support and access to expert symptom & medication management, and care will be coordinated with the patient’s Cardiologists. This collaboration combines the expertise of the AHA, and nearly 75 of the nation’s leading nonprofit palliative and hospice care organizations - combining best practices to provide the best symptom management and support.
