Trellis Supportive Care provides hospice support, palliative care, and grief counseling services to patients and their families in 13 counties. Our dedicated staff members and volunteers visit private residences, nursing homes, and other facilities throughout northwest North Carolina.
We understand that no two patients are alike, and neither is their care.
We listen to the goals and wishes of each patient so that we can create a personalized care plan. And we make every effort to be sure that caregivers know we are here to support them too.
Different illnesses have different effects and symptoms — so we focus on each illness category to be sure we are providing the very best care to ease those symptoms and discomfort, as well as ease anxiety and sadness.
